Mitigation strategy

1. Establish a Comprehensive AI Governance and Accountability Framework To counteract the risk of regulatory and management failures, organizations must institute a formal, cross-functional AI governance framework, such as the NIST AI Risk Management Framework. This framework must clearly define the roles, responsibilities, and escalation paths for all stakeholders—including compliance, technical, and executive teams—to eliminate "diffused accountability" and ensure clear ownership of AI system risks throughout the entire lifecycle. 2. Institute Continuous and Adaptive Risk Monitoring Systems Given that AI systems are non-static and prone to model drift or decay, traditional, periodic risk assessments are insufficient. Mitigation requires deploying continuous monitoring mechanisms that track model performance, data quality, and compliance metrics in real-time. This adaptive oversight should integrate incident logging, periodic stress testing, and vulnerability management to prevent operational failures from escalating into systemic harms. 3. Mandate Cross-Functional Human-in-the-Loop (HITL) Oversight and Validation To prevent operational failures stemming from over-reliance on automated outputs, human oversight must be formalized. This involves mandating Human-in-the-Loop (HITL) processes for critical decisions, establishing mechanisms for human review and validation of AI outputs in high-impact contexts, and ensuring human operators possess the authority and training to override or disengage the AI system when anomalous behavior or potential harm is detected.
